Please Read If you are a new client, or this is your first visit at my new location, please select the "New Client" appointment. You will be asked for your insurance information and you will need to fill out the intake form.

I am accepting Providence, Regence Blue Cross Blue Shield, and MVA insurance only.

A 24-hour notice is required for cancellations. There is a 50% cancellation fee for appointments not canceled within this time frame. Your insurance must be verified BEFORE your first visit. You will be required to pay the full service price at the time of your appointment if your insurance information isn't provided and verified beforehand.
